There is a small cost associated with obtaining a completed FME1 form (aka 'Management Pack') from Little Midhurst Management Limited.
The cost is currently £20.00.
The timescale for return of a completed FME1 is currently an average of 10 days, dependent upon how quickly payment is cleared and the availability of LMML representatives to complete the work.
The process is as follows:

Download accompanying documents using the link below. If any others are requested they will be sent by email with the completed FME1.
On receipt of cleared funds, your request will be processed as soon as possible. A PDF copy of the completed FME1 will be emailed to the address provided on the request form.
